OPR office will be moving to modified operations 3/23/2020.
To keep our staff and the community safe, the OPR team is following OHSU and CDC guidelines on teleworking and social distancing. We will make every effort to minimize any interruption to our usual services.
POLST forms will be entered into the Registry as usual.
POLST information will still be available to EMS and healthcare providers in an emergent situation through our emergency hotline.
OPR business phone lines will remain open during our business hours but our on-site staff will be drastically reduced. If we are taking another call, please leave a voicemail and we will get back with you as soon as possible.
Confirmation packets are still being mailed out. Please expect a letter from us 2-4 weeks after POLST form submission.
