How it works:
- For the first time ever, Oregon healthcare providers will have direct access to OPR’s database
- The portal is a stand-alone web portal with EHR integration capabilities
- Two levels of access:
- Read only: Personnel involved in patient care but not a valid POLST form signer (social workers, nurses, healthcare administrators).
- Full access to POLST form database
- POLST form creator: MD, DO, NP, PA, ND.
- Full database search access and ability to create new ePOLST forms
- Read only: Personnel involved in patient care but not a valid POLST form signer (social workers, nurses, healthcare administrators).
How to enroll:
- Starting in July of 2021, we’ll begin a rolling enrollment (2-3 organizations per week)
- Each healthcare facility will need to designate an administrator to assign user access
- The administrator will be in charge of annual assessment of users and access
- Fill out the enrollment form HERE and requests will be processed in the order they are received.
